Future Care Planning Resource Centre
The Future Care Planning Resource Centre (FCPRC) serves as a hub that builds an ecosystem of support to strengthen sector capabilities through the provision of training and upskilling of social service agencies (SSAs) and community partners.
Rooted in the five key domains of Future Care Planning — meaningful engagement, relationships, housing and daily living, medical and healthcare, and financial — the Centre offers resources and expertise, as well as works alongside community partners to empower persons with disabilities and their caregivers to plan early and confidently for their long-term care.
The FCPRC is an initiative by SG Enable and is located in the Vista block at Enabling Village.
Our Role as a National Integrator and Ecosystem Builder
SG Enable plays a central role in developing the sector’s structures, capability and operating frameworks as well as supporting social services professionals, caregivers and persons with disabilities. In the area of future care planning, we serve as an integrator to strengthen coordination, close gaps, and encourage stakeholders to take a holistic, long-term approach across the ecosystem.
Features & Services
Hub-and-Spoke Sector Development
The FCPRC will be supported by an Advisory Panel to co-lead and co-develop future care planning efforts with SSAs and community partners. By working together, SG Enable and sector partners help develop viable options and pathways for the sector and clients.
Operating on a hub-and-spoke model, the FCPRC works with the natural touchpoints that serve persons with disabilities and their caregivers. Through lifting the capabilities of the sector professionals, and developing shared resources and common standards, a more coordinated and consistent approach to future care planning ensures that families are better guided.
Collaborative Space for Training and Resources
FCPRC equips sector professionals in guiding clients and their families through the future care planning process. It fosters cross-sector partnerships to collaborate on initiatives, share best practices and update on emerging developments.
It also drives the development of planning guides, toolkits and playbooks, providing sector professionals with practical and accessible resources to initiate the future care planning journey with their clients.
Direct Support for Sector Professionals with Complex Cases
SG Enable’s trained Case Managers and its Advisory Panel, with expertise in areas related to future care planning (e.g., legal, housing, financial), will support SSAs and community partners on complex cases. This ensures sector professionals receive adequate support to guide families with more challenging circumstances.
Caregiver Engagement through Community Partners
FCPRC works with SSAs and community partners to raise public awareness on the importance of future care planning, by conducting workshops and engagement sessions. This encourages early planning for the long-term care needs of persons with disabilities and their caregivers. Training is also conducted for caregivers to become future care planning advocates.
