Future Care Planning Resource Centre
- Who is this for? Social service agencies (SSAs), community partners, and sector professionals.
- What does it include? Resources and guidance for professionals to help persons with disabilities and their caregivers with future care planning, including information on future care-planning areas (e.g. legal, housing, financial), workshops and engagement sessions, and direct support for complex cases.

The Future Care Planning Resource Centre (FCPRC) is an SG Enable initiative. It builds a more coordinated support ecosystem to strengthen the capabilities of SSAs and community partners to help persons with disabilities and their caregivers plan early and confidently for their long-term care.
FCPRC provides training, resources, and expertise to help professionals guide families across key areas such as meaningful engagement, relationships, housing and daily living, medical and healthcare, and financial.
Special Needs Trust (SNT)
- Who is this for? Persons with disabilities and their caregivers who want to plan for their loved one’s long-term financial care needs.
- What does it include? Affordable trust service to safeguard funds set aside by caregivers for persons with special needs, along with support from Case Managers of the SG Enable Future Care Planning team to develop personalised financial care plans.

Through the SNT’s affordable trust service, caregivers can secure the long-term financial well-being of their loved ones with disabilities. The principal value of the trust funds is quaranteed by the government, with 90-100% fees subsidised by the Ministry of Social and Family Development.
This gives caregivers greater peace of mind by ensuring their loved one's financial and care arrangements continue to be managed according to their wishes, even if they are no longer able to provide care themselves.
Enabling Services Hub

Enabling Services Hubs (ESHs) by SG Enable and partners are community-based touchpoints that bring disability support services closer to the homes of persons with disabilities and their caregivers.
ESHs provide persons with disabilities with easier access to coordinated information and referral services, as well as offer programmes and inclusive activities that build skills and encourage community participation. Caregivers are also connected to relevant support. This enables them to be meaningfully engaged within an inclusive community.
