Home Based Care Services (HBCS)
HBCS helps adult with disabilities to live in the community and support caregivers in their caregiving role.
Overview of Programme
HBCS provides alternative care support for adults with disabilities, with the aim of keeping them in the community for as long as possible. HBCS provide services such as therapy, personal hygiene care, housekeeping and medication reminders.
HBCS is suitable for adults with disabilities who lack the functional or cognitive ability to perform any of the Home-based Care Services components.
SG Enable provides referral to government-aided HBCS providers in Singapore.
Who Can Apply
Applicants should be:
1. 16 years old and above#^
2. Singapore Citizens or Permanent Residents1
3. Diagnosed with congenital disabilities of the following nature:
- Intellectual (IQ level of less than 70)
- Sensory (Hearing/Visual impairment)
- Developmental (e.g. Autism Spectrum Disorder)
- A combination of two or more of the above
*Applicants with acquired disabilities will be considered on a case-by-case basis.
4. Assessed to require assistance in his/her Activity of Daily Living (ADL) or Instrumental Activity of Daily Living (IADL), and lack the functional or cognitive ability to perform any of the Home-based Care Services components.
Applicants may not be eligible for any of the following reasons:
- They have untreated infectious diseases
- They are suffering from serious psychotic disorders and/or serious behavioural problems that require them to be under close individual supervision or nursing care.
Applicants should require six hours of HBCS, including at least 45 or 75 minutes of therapy services per week, based on their Individual Care Plan2 (ICP).
#Special needs children of SPED School-going age should be
encouraged and assisted to remain in School for as long as possible. Parents of
SPED graduands may approach their School to discuss the transition plans of the
children prior graduating from School.
^Persons with disabilities who are below age 21 would need the
consent of parents or legal guardians for their application.
1 For Permanent Residents, at least one immediate family member, i.e. parent or sibling, must be a Singapore Citizen.
2 ICP would be developed by the service providers with inputs from applicant and their caregiver after the screening assessment.
How to Apply
Please approach a social worker from a touchpoint (e.g. Voluntary Welfare Organisation (VWO), hospital, etc.) for assistance in submitting an application for HBCS.

Upon receiving the referral, SG Enable will contact you to provide information to help you make an informed decision on your choice of service.
Acceptance into the programme at the preferred home is subject to the availability of vacancies and approval of the respective home management.
Application Processing Time
SG Enable will contact you within 10 working days upon receiving the completed application forms from the touchpoint. SG Enable will also update the social worker who assisted in the application on the status of the application.
